Gnats 4658: Flakiness opening "Create Passthrough Accounts" dialog

Revised Title: "Create Passthrough Accounts" in Accounts Manager 
 dialog is inoperable after first use.

For no good reason, the "Passthrough Accounts..." menu action was 
configured as a radio button.  The indicated code, below, needed to 
be removed:
